Her concern was her monolid. Her eye-opening muscle (levator muscle) had great strength. The most noticeable feature was her strong Mongolian folds, which are a muscle running from upper to lower eyelids. When the Mongolian folds are too tight, the shape of the double eyelids after surgery may not be natural or ideal unless the muscle is released. 

We recommended double eyelid surgery and epicanthoplasty. We subtly lengthened the inner corners of the eyes, and made sure to release the Mongolian folds to create a natural shape. As a result, natural as well as noticeable in-out folds were achieved.
